If you wish to see me, I require accurate and real personal information, including specific work-related details, strictly for screening purposes. Please rest assured that no one is ever contacted—all information is kept confidential and is simply cross-referenced online so I can verify who I am meeting.

NEW CLIENT REQUIREMENTS

New clients must provide two provider references.
If you do not have references, you must:

• Complete the screening form in its entirety

• Provide a valid photo ID (driver’s license, state ID, or business card)
